Responsibilities:
1.
Work with the Director, Case Manager, and/or family members to fulfill the Plan of Care and coordinate services with other agencies to ensure maximum service coverage for the client.
2.
Health Monitor - includes monitoring blood pressure, pulse, lungs, heart, height, and weight.
Make doctor's appointments when necessary and report doctor consultation in progress notes.
Also monitor diabetic clients' blood sugar.
Nail and foot care might be needed.
3.
Responsible for the coordination of transportation and escorting of patients from home to the center.
4.
Responsible for telephone reassurance each morning before pick up with families as needed.
5.
Responsible to assist with Nutrition Program which includes:
a.
Prepare snacks b.
Lunches c.
Kitchen Clean-up d.
Monitor and record daily meal intake 6.
Record Daily Attendance with sign in and out times.
7.
Responsible for assisting clients with the activities of daily living:
a.
Personal care and grooming b.
Dressing c.
Bathroom Assistance 8.
Assist with exercise and activity program 9.
Assist with maintenance of facility, cleaning of rest rooms, trash take-out.
10.
Respect privacy and hold in confidence all information obtained while providing services.
Education and Experience
Qualifications:
The position prefers an aide with experience working with intellectually disabled adults and the aging population.
