00/hour starting pay - Full Time Position Essential Duties and
Responsibilities:
Work with the Director, Case Manager, and/or family members to fulfill the Plan of Care and coordinate services with other agencies to ensure maximum service coverage for the client.
Health Monitor - includes monitoring blood pressure, pulse, lungs, heart, height, and weight.
Make doctor's appointments when necessary and report doctor consultation in progress notes.
Also monitor diabetic clients' blood sugar.
Nail and foot care might be needed.
Set up medicines for clients.
Responsible for the coordination of transportation and escorting of patients from home to the center.
Responsible for telephone reassurance each morning before pick up with families as needed.
Responsible to assist with Nutrition Program which includes:
Prepare snacks Lunches Kitchen Clean-up Monitor and record daily meal intake Record Daily Attendance with sign in and out times.
Responsible for assisting clients with the activities of daily living:
Personal care and grooming Dressing Bathroom Assistance Assist with exercise and activity program Assist with maintenance of facility, cleaning of restrooms, trash take-out.
Respect privacy and hold in confidence all information obtained while providing services.
Education and Experience
Qualifications:
The position requires LPN or RN certification with a valid KY Nursing License and experience working with intellectually disabled adults and the aging population.
